Admission Information
The information on these forms will assist The Prince William Medical Center's Assisted Living Center in meeting your needs. We encourage you to complete the forms and return them to our Admissions Office, as soon as possible. Upon receiving the completed forms, we will schedule an appointment with you so that we can discuss our facilities and services at greater length and detail.
People with a wide variance of needs call The Assisted Living Center their home. Set up an appointment with us to discuss what your needs may be to determine if The Assisted Living Center is the right choice for you. Once that decision is made and an apartment is chosen, a refundable deposit is required to secure your personal reservation.

Pricing Information
Each variable-sized apartment comes with local and long distance telephone service, weekly housekeeping services, cable TV service, three meals and snacks daily, access to all on-campus activities, large closet space, a kitchenette, individually-controlled heating and air conditioning units, and a large bathroom with accommodating safety shower.
